Jack Polyak, age 84, passed away from a brief illness at Presence St. Joseph Medical Center on Saturday, January 5, 2019 with his loving family by his side.
Jack is survived by his beloved wife of almost 62 years, Dorothy (nee Bazaar) Polyak; loving daughter, Diane (the late Dennis) Adkisson; grandsons, Edward Adkisson and Kenneth (Autumn) Adkisson; great-grandsons, Caius and Avery Adkisson; sisters, Anne Davia and Dr. Barbara Conway; and numerous nieces and nephews.
Preceded in death by his parents, Mirko and Vincenca (nee Tomich) Polyak; siblings, Mary Cvetkovich, Joseph Polyak, Thomas Polyak, Anthony Polyak, Michael Polyak, Catherine Sloka, and Margaret Slabozeski.
Jack was born in Lockport Township, IL. He graduated from Lockport Township High School class of 1953. He later went on to marry the love of his life Dorothy (nee Bazaar) Polyak. Jack attended Governor State University. He was awarded 88 credit hours from Governor State University for his on the job construction experience with several major companies. Jack received several safety awards for an accident free work environment as well as awards for excellences in bringing jobs in on time with high quality. Jack started his career in the family construction business and later went on to work in the industrial construction and engineering companies, such as, Fluor Construction Company, Davy McKee, Brown & Root Construction, Jacob’s Construction, Burns & McDonnell, F.A. Wilhelm Construction Co., Inc. and Abbott Laboratories. During his years of construction employment, Jack held many positions in construction management as chief engineer, superintendent, and project manager. He was known for his ability to solve complex construction problems. Jack declined overseas job placement to Saudi Arabia and elsewhere in the Middle East so that he could be with his family, whom he loved and cherished. Jack’s family always joined him in his travels around the United States. He will be dearly missed by all who knew him.
Memorials in Jack’s name may be made to charities of the donor’s choice.
The family invites relatives and friends to gather on Thursday, January 10, 2019 at St. Mary Nativity Cemetery for a graveside chapel service at 10:30 a.m. Per Jack’s wishes cremation rites have been accorded. There will be no funeral home visitation.
