Alfonso Diaz "" Poncho"" Age 28, at rest Wednesday November 4, 2009 at Provena St. Joseph Medical Center. Survived by his beloved wife of nine years Kathleen Marie (nee Nowak) Diaz, Loving father of Michael, Clarissa, Ashley, James, Alfonso Jr., Isaiah and Aaliyah, Dear parents Sacramento Diaz and Rebecca Martinez and Maria Diaz, father and mother-in-law Thomas and Carol Nowak, brothers and sisters, Sacramento Jr. (Lidia), Sofia (Gilbert), Angelina (Efrain), Elizabeth, Roberto, Lucy, Imelda, and Selena, also survived by grandparents, aunts, uncles, nieces, nephews, sisters and brothers-in-law, cousins and friends.Preceded in death by his brother Junior. Alfonso was born in Joliet and a lifelong resident. In lieu of flowers donations to be used according to the families wishes would be appreciated.
